Biopsy/puncture under Ultrasound guidance, how can it be so "easy" and "accurate" with single puncture ?

The difficulties are described as below :
Difficult point 1
During out-plane puncture , the doctor is unable to visualize the complete puncture needle, so it is difficult to estimate the position and angle of the puncture needle reaching the target. The puncture technique and experience are highly required.
Difficulty Point 2
During in-plane puncture, when the needle and the probe surface are in the same plane, the needle can be completely displayed, but it is difficult for the doctor to ensure that the needle and the probe surface are completely in the same plane during the puncture.
